All renewals and CE reports will be reviewed prior to issuance of a renewal certificate.

Certificates will be mailed to your correspondence address. So please complete the correspondence address field to send your license renewal and any other mail we may send you where you want it to be sent.

If you answer yes to any of the questions on pages 4 or 5, you are required to follow up with written explanation and/or documentation by mail, email or fax. Your license will be delayed until such explanation and/or documentation is received.

If you are in default with Worker’s Compensation or Unemployment Compensation, your license renewal will be delayed.

Also, the Board, in cooperation with the West Virginia University School of Dentistry and West Virginia Department of Health & Human Resources, Oral Health Program, requests each Dentist and Dental Hygienist complete and return to the Board office a dental census.  The purpose is to match new graduates with dental practitioners actively seeking an associate or dental hygienist and gather information on clinically active dentists and hygienists by county.  The census was created to ensure no personal identifiers are included in your response so your answers will be confidential.   Please click the links below to print and complete the census that applies to you and mail to the Board office.  Your cooperation is appreciated.
